ON – Parent Not Paid Process

Please fill out this form to request documents to initiate the Parent Not Paid process.

This form is to be completed only by adult members of Girl Guides of Canada.

Once the form is completed, the Guider will receive an email outlining the steps and required documents needed for the Parent Not Paid process including:

• Two payment letters sent by mail to the family (10 days apart)
• Parent Not Paid Procedure Form
• Record of Parent Signature
• Record of Parent Discussions
